What happened was I took the wreath downstairs to hang. When I got back upstairs I found that rogue leaf on the desk. It came off up behind the bow. For some reason my leafs are not attaching tightly. I have them all the way on, but they are feeling loose and this is with me not doing anything to them. I have glued almost every one. Which I am ok with.

It’s strange that they’re so loose. Luckily, glue is our best friend 
I had to glue some of mine too. The 2 pieces underneath the bow came off too,l.
